In reality, your "tough love" ideas have already proven not to to work.
Right now, the Reagan law MANDATES hospitals treat anyone who has a need for care, including illegals, regardless of their ability to pay. You can't MANDATE that hospitals treat people who can't pay, without finding a more EFFECIENT way to cover the cost of unpaid healthcare. The Reagan MANDATE is, in essence, an entittlement. But, as usual, it's a GOP entittlement/mandate they never paid for. They simply put it on the credit card.
Right now if the uninsured has chest pains, they go to the hospital ER and get checked out. The ER is the most expensive place to get treated. That costs of unpaid ER treatment is passed on to those of us who pay. We're already paying for their healthcare. But were doing it in the most in-effecient and expensive manner. Paying for treatment recieved in the ER is the worse thing we could be doing. Get these people some insurance, so they can stop going to the ER. It will cost us MUCH less.
The only other alternative is to end the Reagan mandate for hospitals to treat anyone who needs care. Then we can just let people die in the streets like a 3rd world country.
