I get that with Cox. It seems that when they changed "loose fittings violation from 3 points to 11 points everyone started failing QCs with an 89%. I've been taking pictures of my wrench on EVERY connector. I've actually caught a few QC techs saying I left loose connectors, and provided my pictures of proof and the charge back was removed. It's a pain in the ass but I take at least 10 pictures of every job I do. It's saved my ass many times. I use my HTC Hero to take the pictures and just upload them to picassa with my Google account. It will Geotag all the pictures with GPS coordinates and a time stamp. It even shows the location in a Google Maps pane. It takes a few extra minutes at the job, but when a job comes into question I just email the link and let the pictures do the talking.
