QAM levels

When your digital channels are 256 qam, should these levels at the headend be 3 or 6 dbmv down from the last analog channel. ex: if ch. 78 (547.23 mhz) is at 16dbmv should ch. 79 (555 mhz) be 13 or 10 dbmv?
