Sounds to me like an ingress problem... Though, you may or may not have unity gain on your return path. There are a couple of MSO's who don't see the point of sweeping the return to the terminator and others who think sweeping the return on the actives is money foolishly spent. Adelphia, here, suggests that the tombstone value is close enough. Another phenomenon, that might cause an upset of unity gain is that some carpet installer may have been handed a Calan and the keys to a bucket truck as opposed to a sweeper with experience. Lastly, perhaps your system is an example why we need to do a scheduled preventative maintanence sweep on all plant.
