Sounds like all of the things we would expect have been checked but some times it is the little things. I had the same broblem a few months back did all the same steps you have then I started over and looked at every thing and noticed the only thing not changed was the block in the patch pannel, that had never been touched mind you. I removed both fibers cleaned and checked with a scope replaced the block and ta da no more noise.
