i was splicing for a while and seen some things that would promote don't remove till all tails are hanging
there was a working fiber in the blue tube going one direction it hit the splice and was turned around in the green tube in the same cable the oposite direction and it was not documented on any prints
with that being said it would only need to be in one splice to knock something out
