My first question is are you using a dead zone/suppressor/fiber ring? If you are not, you should be. It is the only way to get an accurate launch loss with an OTDR. Secondly, have you confirmed that the connector on the OTDR is clean, not damaged? I try to mark my test jumpers with OTDR & FDP. This way the end I plug into the OTDR is not picking up the debris from numerous FDPs. It helps to reduce the chance of getting something nasty on my OTDR connector if I forget to clean it before plugging in.
