I have seen this and can be several causes. If it is on the shield it usually traces back to a power problem. Bad Neutral or loss of ground. If it is on the center conductor it is usually caused by a faulty device. Things I have come across: Bad Isolation in the TV's tuner/VCR/etc. In these cases the device needs removed from service and customer needs to realize it needs fixed. This is also a fire hazard. One time I did find a radio shack style house amp that was putting AC back on center conductor.
