1- Who controls your daily work (where you work-quit time-start time)
2- Do you report to work at the same location every day
3- Do you submit daily production reports
4- Who controls your week end work
5- Is your daily work inspected
6- How do you pay your helper, employee's or sub also
7- Who issues the material to you
8- Who provides insurance for trucks as with G/L insurance
9- Do you pay work comp for your helpers
10-Do you keep mileage reports and receipts for fuel, food, motels, tools, road expense
11-Do you have a sub contract agreement
12-Do you have a FID number or just SS#
13-Do you pay quaterly taxes
14-Do you have a office
15-Do you charge off telephone, cell, postage, supplies, utilities, and such
If the General/Prime contractor gives you a set of prints for a phase-node and issues you all required material and tells you to come back when done.....Then your a Sub Contractor, If he controlls your daily activities..then your an employee
