1st yes I would contact the Dial811 / Locates and cancel that ticket under your name,
Contact the Utilities to notify them.
I would then sit down and have a heart to heart with the PM or the Supervisor whom issues the work and see whats up when you leave that meeting you will then have your answer as to how the project will go. If you have a good feeling then I would give it 1 more shot but if it happens again I would then consider another project.
The way I run my projects is I have a job# and your john hancock is next to that job# to show its been issued out to avoid those problems.
