As an independent contractor draw up any form you desire if it clearly states what the customer is signing off.
I had a cust with the same dish issue that wanted a pole after it was okd to house mount with a sign off by the cust
He didnt like it when i told him it would be 100.00 to change it but his wife had already signed ok for dish placement
