Sounds like a plant problem. Could be any of a number of things. I would probably rule out a headend issue unless there is other complaints of the same thing, but it doesnt sound like it.
Your levels seem fine, I would look for it to be a possible issue with upstream CPD, impulse or an upstream SNR issue( either MER or BER)
