Did you check to see if the HSI was working off the MTA? Could be a balancing issue with the account and the HSI on the mta is on and Dispatch didn't pick up on it, or at some point the account balancing was off and Dispatch corrected something and at the same time they had you swap out the modem and the wrong MAC address is in the system (MAC from the first one you swapped out or if the customer had previous equipment that modems mac).In a nut shell verify the MAC address of the equipment the HSI is supposed to be running off of.
