Delay between sound.

I had a tech come to me today about a job he did yesterday.  the sound came threw the tv but you could visually see that the lips moving where delayed.  almost like an old chineese movie formated to english.  The box used was a comcast DTA and the line was 59.  Other than the box just being slow what else could cause this?  I have never seen this problem before unless it was on an Hd box, never on a SD or DTA.  Thanks in advanced.
