621 MHz is OK for QAM channel but 670 MHz is not the center frequency, the center frequency is : 669 MHz (ch: 103)
Since you are suppose to use the center frequency of a NTSC channel, this could be your problem.
As far a selecting ch: 95 make sure they are no FM station in that band (93 MHz center frequency +/- 2.5 MHz)
As far a using channel: 2 (57 MHz center frequency) I would also expect some problem with bi-directional filter on the amplifiers. We are using ch: 8 (183 MHz) and it as been a good channel for us.
We are using house amplifier in all of our modem connection, these amplifier are 870 Mhz without return amplification, just feed thru.
