Since you mention both 60V and 90V plant, I presume we are talking about multiple locations. What is common to all the locations effected? Also, I have seen grounding of rebuilt areas, particularly retro, cause some pretty interesting problems.
If you have a location or two that seems to be going through the modules, you might investigate the grounding.
Finally, check the lot numbers on the faulty modules. Is it possibly a bad batch of modules from the factory?
