After reading all the post, I would consider CTB (Composite Triple Beat) probably a high output level somewhere in the cascade of this sector. Look to me like you have reach the crashing point (highest output level possible of an active) On the 77 channel system these horizontal line would show worst on channel 40 to 45 and it would diminish going toward highest or lowest channel.
Low voltage on the last active would also scare me. Today amp. requires around + 30 VAC to work properly. If you have low VAC the IC cannot work properly and give all sort of problem.
