This happens more often than the cable companies will tell the field. When I was in San Diego 2 people died - One from cutting a mid span on the wrong side, the other from not securing his ladder properly on a hill. As we all know, if you as a contractor are responsible for training someone or an in-house tech, your first responsibility is safety to that person because you are the only one he has to learn from. One other thing to tell techs to look out for is any strands by salt water - these will corrode and snap. My sympathy goes out to the techs family.
