Have seen mabye of bonehead things done in cable, one that comes to mind is when I placed a 2" orange duct across the I-15 in California. I informed the cable company not to use orange duct and to order some black duct but they told me to use the orange duct. So I put it up within 2 weeks I was taking it down and placing black duct in it's place, note I had already put the fiber through the duct and spliced it. The city and Caltrans informed the cable company of the eye soar that motorist were having looking at the orange duct across the freeway. That mistake cost the cable company tens of thousands of dollars to have me go back and replace the duct. That is one incident of wasting money on the cable providers part
