Grounding is probably your culprit. In the future, make sure all module retaining bolts are snug, then check and make sure all module lid screws are tight and lastly make sure all the grounding straps are in the module. These are flexible strips of metal that contact the module to the lid. If you are using automatic controls in your amplifier you should have one that contacts the ACM to the module lid. If they are missing they can be added. check with your supply people.
