I gotta read that thing from beginning to end. My last CATV work was about 1990. I just read the section about dish antenna location to satellite installation subs.
From what I see here, either the CATV folks will start doing as a you mentioned and provide phones (read fiber phones & internet) or they will be gone. While not all addresses will ever get a small dish or sat internet, I don't think there will be enough addresses left to keep CATV operating.
I remember looking for small suburban clusters the CATV folks decided not to serve. They skipped the joint trench day (s) and Primestar (me, Pstar sales & installation) rolled them up, followed by DTV & DISH. No trees, free trench for all the drops.........they were just too busy. Wonder how they are liken it now?
