Can't help you with contact numbers , sorry . But , I would suggest contacting the cable commission for your municipality , or county , or the local/state utility commission . Although it may seem like a huge uphill battle , you might also contact the FTC , and the FCC . If he is being charged for the HSI , and they are not delivering the service , they are in violation of several State and Federal laws . If you feel they have been given sufficient time and notice to correct the problem , then , it is now time to go over thier heads . Best of luck .
