Sounds like youve done the normal testing. I have had this problem a few times myself. You may want to check and see if th AC wiring in the house is the problem.

I have had cases where the AC outlet has a fault and causes an IRD to fail. Usually when theres a switch all IRDs will have the problem. But when the fault is not that severe it may only effect 1. What I do is run an extension chord to an outlet of the working irds. If the failing IRD works you know its the AC outlet. You cant fix it, well maybe you can, but you can at least prove to the customer the problem is not due to the install.

You may want to remove your ground and check the IRD, a bad house ground will also cause the problem youre having. Good Luck!!

If you are installing D10 or D11s, if the signal meter sounds on the ird does not change as the signal gets higher, its probably an AC fault.
