It depends on whose equipment you're using. I'm familiar with Motorola headend equipment. Moto uses a device called a Smartstream Encrypter/Modulator (SEM). It's quite the piece of equipment.
Used as a QAM modulator, it has inputs for up eight ASI digital streams, each with up to ten channels each. The output ports (4) each put out two adjacent QAM carriers. They are selectable to any frequency.
Moto's SEM manual has a ton of info, and is fairly easy to read.
