In my experience, when after the 2nd call for hits does not kick you an IP or get you valid for Movies-On-Demand, the error is either with whoever attached the box to the customers account or it could be that the hits are backed-up in the system. Keep an eye on your hit counter to rule this out.
I've found that their error is either with the box NOT being attached to the correct outlet number, the account is not balanced properly, or dispatch did not verify that the services line up is correct.
So, next time you run into a 3rd phone call, ask your dispatcher:
1 - Is the box on the correct outlet?
2 - Is the account balanced properly?
3 - Does the number of boxes on the account match up with the services line up?
This should kick their $11/hour asses in gear.
Now if dispatch verifies all this and the box still isn't fully active, then you have a tech-install problem. My guess would be to check to make sure that your FDC, RDC signal strength is within spec as well as your S/N and EQ-Gain.
If it's a signal strength issue, get back out there in the rain and reconfigure your D-Mark so the box gets some solid signal. You can also run a temp-jumper from the tap to the D-Mark to see if the lateral is bad or not too. And as always....... CHECK YOUR DAMN FITTINGS!!
