That is a 3-4 switch used for dishnetwork. The 3 lines on the left are the inputs. They are feeds from 3 different sats. The 4 lines at the bottom are to the receivers. The 3 ports that are open on the right are there if you need to cascade switches for more receivers.
As far as that failing a QC I can't see in that picture a reason to fail. Yeah sure it could have been done differently but evey person that looks at it will have an opinion on how it could have been done. The main question still remains...does it pass the specs that are required by Dish Network. Hard for me to tell from that pic but I have seen alot worse make the grade.
