I live by that accident scene and was also working at that intersection on mainline maintenance. The problem there was that the tap was mid-span due to a pole being removed and placed about 50 feet away from it's original location. Therefore the tap and loop were below telco. as he said. At the pole it was above telco and below the second cable system in our area. So not only was it a mid-span tap but it was also in the newly built intersection. Pretty hairy stuff to due alone. The tap was mid-span for about 2 years before the accident. Corrected directly after the accident. I lashed a new piece of cable there and it took 2 years to splice it! What a shame.
