Fiber problem


I would like to know if any one could figure this one out. I just got done with a fixing some breaks in: 1. Splice case 2. Patch panel in a tray. Both of these splices were in these little splice holders then glued in with a silicon goop. As it turns out I was fixing a fix. Now my question is, did the fiber break due to not having mechanical support, or was it due to a bad splice and then extreme hot and cold with this hardened goop splice holder? I use a heat shrink with a metal bar in it. This has done me good but will it have this problem again? Also to the best of their knowledge no one has touched anything and it is in the nowheres up in the mountains and I am stumped how a splice can just go bad.(confused)(bigcry)
