Sounds like it may be more local to the individual customer, Have you checked the noise and ingress on the coax at the customers home? I recommend checking the drop at the modem, Then the tap, Then at each amp then the node. Also if you have an sda 5000 inject a return signal at the customers modem location and measure that single at the headend see what your loss is. I had a node where some of the customers had to much loss in the system on the return path, The modem would throttle to the max output and work part of the time but if an agc or thermal would not compensate for temperature changes the modem would drop out.
