Your build was tested to the MST prior to handover for turn up. It would appear bi-directionals were not completed back to the Cross-Connect cabinet. MST's have a defect rate of about 1% at a port, likely your issue.
From your comment it doesnt appear your employer or the MSO are that concerned with correcting deficiencies quickly. Otherwise they'd spend $3500 on a last mile OTDR for you as well as allow you to complete re-entries to correct high loss or incorrect splicing.
Given the industry I wouldn't waste a dollar of your own money on equipment unless you suspect there is some way to monetize it in the near future (i.e. working for your self).
