It's called being clueless!  They had very poor and inexperienced leadership from the very top at the executive level all the way down to site level management that would manipulate their clients and falsify documentation/billing.  Their payscale was disproportionate as well between entry level to senior level veteran technicians.  Top that off with a "proprietary technology" that could only satisfy a very small niche market and had no realistic widespread application in the continental U.S. and you get the end result of a TOTAL COLLAPSE.
