hit me with an address or fax number and I'll send you one that I made up back in 90's, it covers by phase/by node/by print aerial/UG plus associated work.  Also great for as built by node.  I even went on to come up with a computer program that you could use the daily and make an invoice to the customer...by node or Pwr Supply, as with tracking materials.   (-REDACTED-)    I'm retired, don't need the paper work laying around anymore
