I have been in this buisness for 18 years and have seen and or heard of this too much. It is a sad thing. It reminds us, no matter who's fault it is, Don't get so comfortable that we neglect safety. We are brothers in this game. I don't know Tad, but can relate. Survival is very low when this happens. I hope he has a full recovery, if it is possible,  and quickly.
