There is only so much that pre-equalization can correct. If you exceed that threshold, post-equalization MER at the CMTS will fail. The only thing that saves you when a modem reports very low input/output SNR and does not affect the overall US port is attenuation back to the node. There are so many variables to truely answer your question completely. These are the most common assuming everything else is correct. Other things to consider - OMI, RF total power to/from RX, node TX type(FP,DFB, etc...), CMTS configuration, plant alignment for proper TX level, US carrier spacing and modulation, and many other practices and variables.
