SA are junk......  try changing the return pads there are a lot of after market pads or non SA approved accessories that are being used and as crazy as it sounds they are intermittently failing .... If you know this amp is an issue plug into the test port before opening the lid try to confirm if your problem is at the ip or o/p  my best suggestion would be to get rid of  the SA equip
