Yes, I have encountered this. What is the age of the cable / and was the cable installed in conduit / duct. Believe it or not, there was tons of OSP / Indoor-Outdoor FO cabling manufactured a few years ago, that when placed in duct and the duct filled with water, a chemical reaction happened with the flooding compound. It hardens like cement, and when moved the least little bit,will break the fiber.
