In some states its not an option, it is mandatory. In other states its not mandatory for the driver to carry that kind of coverage. But it is mandatory that the carriers offer it. 


However, in no state are tax payers left footing the bill for people who cant afford to have their damaged uninsured cars repaired.  Reagancare/EMTALA mandates hospitals treat uninsured people. When that uninsured person can't pay, the insured, and eventually the US tax payer, becomes responsible for the unpaid bill.
