Almost forgot...

Look to see what type of optical transmitters you are using. You should be using the DFB (Distributive Feedback Laser) that is similar to the old "Feedfoward" technology. These are the best type of lasers for the HFC network with a higher threshold for input level. The other type, FP (Fabre Perot) lasers are cheaper and less effective at reducing "clip" due to it having a lower threshold. The comparason between the 2 would be like a Feedforward amplifier to a standard Push-Pull amp.

After re-reading your post, you made a comment that when you open the node, the problem goes away. I'd look at a dirty fiber connection and/or a bad mini-SMB cable connector/connection. But you also said that you were only seeing the problem in the HE. In this situation, I would look at the "clip" issue in the previous post.
