Have you tried testing link loss with an OPM?  It might be good to get a loss reading and compare it to your estimated OTDR loss reading.  Out of curiosity, what splicer are you using? Do you have access to another OTDR to compare? Be sure to clean connecors and bulkheads. Don't use an alcohol swab and your shirt sleeve.  Use a good quality cleaner.  Canned air can be your friend when used properly.
