If you do any type in cable sooner or later you're going to fall. In my 30 + years in the industry I have cut out on more than one pole, had extension ladders fall from under me, and step ladders kick out. It's part of the job, you all know there are lots of times when you got to do what you got to do to get the job done. Even when you are following all the safety steps accidents happen. All you can do is work as safe as possible and hope that when that accident does happen you don't hurt more than your pride.
