Look at Commscope's website. At one time they had a brief construction "spec" book. There really is not an industry standard. Each system seems to have its own. My own rule of thumb: input loops on all actives and splitters/couplers/power inserters. Output loops on every pole. Do not crowd pole with equipment--error on side of more space than less space. Consider end of preform the "pole" for equipment/loop purposes. Do not hang equipment in slack spans unless both spans are slack. To determine output side of pole with cable going both directions (express and backfeed, for example), backfeed or distribution takes precedence over express, or in older systems, feeder determines loops, not trunk. 8 inches of horizontal cable going into a connector at an active.
Every system is different and you'll get lots of different answers from guys. Many will disagree with what I've posted. It all depends on who taught us and where we learned.
