Don't know about pac bell. But us west built a test system with coax in Boulder during the 90's. It only served a couple of hundred homes.
Also,, a co called Wide Open West, built a system in Lakewood, with fiber backbone, and copper and coax lashed together,in hope of offering video and voice, in the late 90's. Payed good money to build it. Then, four years later, payed good money to wreck it.
