It all has to do with broadcast agreements that the MSO's have to sign with each channel or company. The dictate how much per sub they want and in some cases even where in the MSO's channel line-up their channel will appear. I remember Disney use to demand that if a MSO wanted to carry their channel it had to be in their lowest priced tier, you can't get A&E without taking the History channel all kinds of stuff like that.
Most of the cost of running the video side of a MSO goes to the broadcasters.
If everyone is thinking alike, someone isn't thinking.
