Use that 100' temp drop you made and hook it directly to the modem with no amp at all and see if you still get knocked off. If the problem vanishes and if you can't split before the amp then run a 2nd drop (if possible) for the modem.
If the temp drop still experiences the drop offs then it's an OSP issue as long as your temp drop is constructed without faults.
