I agree with Joe. You just don't know until you put your hands and eyeballs on it. It's like asking, "What causes noise?"
Advice:
If you REALLY want to know how/why you're getting roll off, reference it at the node and move forward through the cascade. Find where it's bad and then back up (divide and conquer) to find the source. Take it apart if it's a passive and temp it if it's a span. Recheck at the previous "bad" demark to confirm repair.
In just one cascade you might find several causes of your own that are completely independent of each other. Good luck.
