Worked for teleprompter in early 80's was rebuilding Sierra Madre Ca. and the system had a ladder truck, the tech was on a service call came down off the ladder to check something in the truck and forgot the ladder was still up and took off. needless to say ripped it right off the truck, so they decided to have a bucket mounted to the bed, got it back looked good, tech was using the first day back and boomed up and out and the platform tipped off the bed and catapulted him 50' through the air , they forgot to bolt the platform to the bed, he ended up with 35 stitches across his forehead and 75 grand compensation from the Company that did the work.
