There is no carrier that will cover every state due to several states that are monopolistic states. Which means you must carry W/C ins from that state.  If I remember correct there are like 4-5 of them. I know for a fact that Ohio is one because I have a policy there. If you 1099 the guys under you then you can claim 0 payroll and its $100/yr (ghost policy) if you do wish to W-2 then it is based like most on your payroll.  Hope this helps.
