I agree with you 100% about the door incident. There is no excuse for not being tied off, the old policy of, "I'm jumping outta this thing if it rolls downhill" doesn't work in the bucket. And the buckets with 1 connection pin are especially dangerous because the pin can shear off or pull itself out, I once had a pin work itself almost all the way out of the boom connection and didn't even notice it until the end of the day, though we were using it for <3 span maintenance. And your right, there will ALWAYS be accidents, but hopefully they stay accidents and not fatalities.
