Digital Green was our contractor when this happened, Miguel still has problems from the fall and most likely always will. The bucket door wasn't latched, the safety chain wasn't used and there was not a harness and lanyard in use (there was a harness and lanyard on the truck), DOUBLE CHECK the door and safety chain, even if you fall out with the harness on its going to hurt.
