I might be wrong so I'll go check it tomorrow, but I was under the impression that the arrow pointed into the direction of the hot leg. Am I wrong there? 3 pad on the input because it's fed by a trunk run. This is a 550 MHz system that was upgraded to 870 MHz, so yeah... it's tight.
